Membership Terms

  • Membership fee will automatically be drafted on the 1st of every month.
  • There is a 3 month initial commitment, you may cancel anytime after that.
  • You may not transfer any account credit or memberships to any other patients.
  • You can designate one family member to utilize your monthly treatment if you can’t make it in, at no additional charge.
  • Must redeem birthday gift in the month of your actual birthday. Offer excludes products.
  • Members are subject to the same cancellation/no show fee.
  • There will be a $25 fee if the card on file is declined at any time.
  • Cancellation request can be submitted in writing a minimum of 30 days prior to your bill date, after the minimum commitment is fulfilled.
