Membership Terms.

Membership fee will automatically be drafted on the 1st of every month.
There is a 3 month initial commitment, you may cancel anytime after that.
You may not transfer any account credit or memberships to any other patients.
You can skip a month two times a year, after the initial 3 month commitment.
You can designate one family member to utilize your monthly treatment if you can’t make it in, at no additional charge.
Must redeem birthday gift in the month of your actual birthday. Offer excludes products.
Members are subject to the same cancellation/no show fee.
There will be a $25 fee if the card on file is declined at any time.
Cancellation request can be submitted in writing a minimum of 30 days prior to your bill date, after the minimum commitment is fulfilled.
